Documentation required for accommodation

• An Application for accomodation
• Certificate of Citizenship
• Birth certificate
• a copy of a valid ID card
• a copy of a valid document of health  insurance for health services in Croatia
• a medical certificate by a general medical practitioner, copies of other important medical    records translated to Croatian language
• Psychiatric findings (if deemed necessary)
• Proof of retirement income
• A decision on guardianship (for persons placed under guardianship)

NOTE:
THE COMMISSION FOR ADMISSION AND DISMISSAL OF USERS WILL CONSIDER THE ADMISSION OF USERS INTO IVANEC CARITAS HOME ONLY ON THE BASIS OF A FULLY COLLECTED DOCUMENTATION!

– If the request is put on a waiting list until the realization of accommodation, it is necessary to report to the social service of the Home once a year.
